An exciting opportunity is available for a Grade 1 Social Worker to join our General Medicine team at Monash Medical Centre, on an ongoing, part time (0.2 EFT) basis. Working alongside experienced Social Workers and a supportive multidisciplinary team, you'll have the opportunity to develop your clinical skills, build confidence in managing complex situations and support patients and families during some of life's most challenging moments.

Whether you're looking to further develop your hospital Social Work experience or take the next step in your clinical career, this is an opportunity to learn, contribute and grow within one of Victoria's largest public health services.

About the Role

As a Grade 1 Social Worker within General Medicine, you'll play an important role in supporting patients and their families as they navigate the emotional, social and practical challenges that can arise during a hospital admission.

You'll provide psychosocial assessments, counselling, advocacy and discharge planning, working closely with patients, families, carers and healthcare professionals to ensure care is coordinated, compassionate and responsive to individual needs. Your role will include:

  • Completing psychosocial assessments and providing appropriate interventions for patients with a range of health and social complexities.
  • Supporting patients and families through challenging circumstances, providing counselling, information and advocacy.
  • Contributing to safe and effective discharge planning, helping patients access appropriate community services and ongoing supports.
  • Working collaboratively with multidisciplinary teams to support patient-centred care and positive health outcomes.
  • Navigating relevant health and community service systems, including My Aged Care, NDIS, family violence, child protection and mental health services.
  • Developing your clinical reasoning, assessment and case management skills through professional supervision and learning opportunities.
  • Participating in quality improvement, research and service development activities.

You'll be part of a collaborative Social Work Department where professional development, shared learning and supporting one another are valued.

About You

You're a compassionate and motivated Social Worker who is passionate about helping people navigate complex health and life circumstances. You enjoy working with people, value the importance of teamwork and are eager to continue developing your clinical knowledge and skills. You understand that every patient has a different story, and you approach your work with empathy, curiosity and a commitment to providing respectful, person-centred care.

To thrive in this role, you'll bring:

  • Sound psychosocial assessment, intervention and care planning skills, with a willingness to continue developing your clinical expertise.
  • An understanding of the social and emotional factors that can influence a person's health, wellbeing and recovery.
  • Strong communication and interpersonal skills, with the ability to build positive relationships with patients, families and colleagues.
  • A collaborative approach to working within a multidisciplinary healthcare team.
  • Good organisational skills and the ability to manage competing priorities in a busy clinical environment.
  • An interest in evidence-based practice, professional development and continuous learning.
  • A commitment to the values and professional standards of Social Work practice.

Most importantly, you're someone who brings compassion to your work, is open to learning and wants to make a positive difference in the lives of the people you support.

Why Join Us?

Because the beginning of your Social Work career should be about more than gaining experience. It should be about feeling supported, building confidence and discovering the difference you can make. At Monash Health, you'll be joining a team that values your contribution, encourages your development and understands the importance of learning from one another.

Here's what you can look forward to:

  • Support as you grow – Access regular professional supervision aligned with AASW standards, guidance from experienced clinicians and opportunities to strengthen your clinical confidence.
