About the Role 
Join our team as a Grade 1 Social Worker on a full time, ongoing basis. This position will work with Renal inpatients / outpatients, the haemodialysis in-centre at Monash Medical Centre at Monash Medical Centre in Clayton.  You will also have the opportunity to work at various satellite dialysis units across Monash Health.
 
 
In this role, you’ll provide evidence-based psychosocial assessment, counselling, advocacy, and discharge planning to support patients and carers across a diverse range of clinical settings. Working as part of a multidisciplinary team, you’ll collaborate with nursing, medical, and allied health colleagues to deliver safe, high-quality, person-centred care.
 
 
We will provide you with the opportunity to build your professional confidence through structured supervision, ongoing learning, and development programs, while contributing to research, quality improvement, and service innovation. About Us 
 
The Social Work Service at Monash Health assists patients and families with psychosocial support and advocacy through assessment, counselling, information provision, access to community resources, and discharge planning. Social Workers function as members of the multidisciplinary team and have a pivotal role where social complexity exists. The Social Work Service operating hours are standard business hours Monday to Friday. A weekend service is provided by roster at Casey, Clayton, Dandenong and Victorian Heart Hospital sites and an on-call service is also provided to the acute sites.

About You 
 
 
You are an enthusiastic and motivated Social Worker who is ready to take the next step in your professional journey. You’re passionate about delivering compassionate, person-centred care and supporting patients, families, and carers through times of change and challenge.
 
 
You bring a strong foundation in social work theory and values, along with developing skills in assessment, counselling, advocacy, and discharge planning. You have a sound understanding and working knowledge of key service systems, including Family Violence, Child Protection, My Aged Care, Transition Care Program, the National Disability Insurance Scheme (NDIS), and Mental Health and the recovery model.
 
 
Your ability to work collaboratively within a multidisciplinary team and communicate effectively helps you build trust and achieve positive outcomes. You demonstrate professionalism, curiosity, and a commitment to continuous learning — embracing supervision and feedback as part of your growth as a clinician. Above all, you are driven by integrity, respect, and a genuine desire to make a meaningful difference in people’s lives.

 

Qualification & licence requirements include: 

 

  • Bachelor of Social Work or Masters Social Work Qualifying OR Master of Social Work, OR equivalent as assessed and approved by the Australian Association of Social Workers (AASW)
  • Eligible for ordinary membership of AASW if overseas trained (http://www.aasw.asn.au/)
  • A current Victorian Driver’s Licence
