About the Role
 
We are looking for a dynamic Grade 1 Social Worker to join our innovative team on a full time, fixed term basis (until July 2026) to support our patients to receive the best clinical care at home. With an office base of Monash Medical Centre, in Clayton, you will conduct face to face, and/or Telehealth Social Work assessments and intervention to patients who are admitted to the service. 
 
Responsibilities will include: 
 
  • Providing a high standard of client assessment, intervention and coordination of services to facilitate safe, effective and person-centred care
  • The planning, development, quality and evaluation of evidence-based Social Work service across the continuum of care
  • Participating in the development of a collaborative inter-professional team approach to achieve optimal patient care and outcomes
  • Assuming responsibility for relevant administrative portfolios as directed by the Allied Health and Psychology Manager or the Grade 3 Social Worker
  • Participating in clinical supervision
 
 
 This position works Monday to Friday, 8.00AM – 4.30PM.

Monash at Home (ARC) provides an alternative to inpatient care, and providing safe and high-quality home-based services for individuals with complex medical, functional and cognitive issues, who require comprehensive, medically led multidisciplinary assessment and intervention within the comfort of their own home. The team consists of Medical, Nursing, Allied Health, Pharmacy, Psychology and Administration staff and is a 40 bed services that covers the Monash Health catchment.  
 
 
About You
 
 
You are a Social Worker with a genuine passion and interest in working with patients within a home environment, combined with proven experience and effective assessment and intervention skills. You have proven ability to work both independently and collaboratively and readily build strong working relationships with key stakeholders in the delivery of client centred care. Your strong communication skills and empathetic approach enable you to build trust and rapport with clients, families, and multidisciplinary teams alike.
 
 
You are described as someone who is adaptable, organised, and confident in navigating the challenges of community-based work, including managing a diverse caseload and supporting clients with varying levels of need.
