About the Role 
 
We are seeking a Grade 2 Social Worker to join our growing Social Work team at Monash Health, based at Monash Medical Centre (MMC). This is a full-time, fixed-term position until 27 September 2026 (parental leave cover).
 
In this role, you will manage a clinical caseload within designated specialty areas, maintaining and further developing your clinical expertise. You will apply consolidated psychosocial assessment, risk assessment, and care planning skills, using evidence-informed practice to support patients and families experiencing complex health and social needs.
 
The position will primarily support the Gastroenterology Medicine, General Surgery and Vascular units, with opportunities to contribute across other specialty areas as required. Key areas of focus include complex discharge planning, adjustment to illness, bereavement support, family support, and multidisciplinary collaboration to ensure safe and effective care transitions.
 
You will demonstrate a strong working knowledge of relevant service systems, including Family Violence, Child Protection, My Aged Care, the Transition Care Program, the National Disability Insurance Scheme (NDIS), and Mental Health services, including recovery-oriented practice. This knowledge is essential to delivering high-quality, evidence-based social work services.
 
About Us 
 
The Social Work Service at Monash Health assists patients and families with psychosocial support and advocacy through assessment, counselling, information provision, access to community resources, and discharge planning. Social Workers function as members of the multidisciplinary team and have a pivotal role where social complexity exists. The Social Work Service operating hours are standard business hours Monday to Friday. A weekend service is provided by roster at Casey, Clayton, Dandenong and Victorian Heart Hospital sites and an on-call service is also provided to the acute sites.
 
About You 
 
You are an experienced Grade 2 Social Worker with strong psychosocial assessment, risk assessment and discharge planning skills, and a commitment to evidence-based practice. You are confident managing a clinical caseload and providing specialised support to patients and families experiencing complex health and social needs.
 
You bring demonstrated experience working collaboratively within a multidisciplinary team across inpatient and community settings, and you communicate effectively with patients, families and stakeholders.
 
You demonstrate:
  • Advanced skills in complex psychosocial assessment and care planning
  • A sound working knowledge of relevant service systems, including Family Violence, Child Protection, My Aged Care, Transition Care Program, NDIS and Mental Health (recovery-oriented practice)
  • The ability to provide a specialised clinical service within designated units
  • Initiative in developing leadership capability and contributing to quality improvement, project and research activities
  • Professional accountability in line with AASW standards and ethical practice

Qualification & licence requirements include: 
 
  • Bachelor of Social Work or Masters Social Work Qualifying OR Master of Social Work, OR equivalent as assessed and approved by the Australian Association of Social Workers (AASW)
  • Eligible for ordinary membership of AASW if overseas trained (http://www.aasw.asn.au/)
  • A current Victorian Driver’s Licence
