About the Role 

 

Social Worker, Grade 2 | Permanent Full Time (1.0 EFT)  | Victorian Heart Hospital 

 

An exciting opportunity has become available for an experienced Grade 2 Social Worker to join the Social Work Service at the Victorian Heart Hospital (VHH).

 

Working within Monash Heart, Cardiothoracic and Ambulatory Care services, you will play a key role in providing high-quality psychosocial assessment, support and intervention to patients and their families throughout their treatment journey. This onsite position offers the opportunity to work closely with a highly specialised multidisciplinary team while contributing to exceptional patient-centred care.

 

You will be responsible for triaging referrals, conducting comprehensive psychosocial and risk assessments, providing emotional support, facilitating access to services and resources, and collaborating with internal and external stakeholders to achieve the best possible outcomes for patients.

The Social Work Service at Monash Health provides psychosocial support and advocacy across acute, subacute and specialist services. Social Workers are integral members of multidisciplinary teams, supporting patients and families through assessment, counselling, discharge planning, information provision and connection to community resources.

About You 

 

 

You are an experienced and motivated Social Worker who is passionate about delivering evidence-based, patient-centred care within a specialist healthcare setting. You will bring:

 

  • Strong psychosocial assessment, risk assessment and care planning skills
  • Experience managing a clinical caseload in a healthcare setting
  • Excellent communication and stakeholder engagement skills
  • A collaborative approach to multidisciplinary teamwork
  • Knowledge of key service systems including Family Violence, Child Protection, My Aged Care, Transition Care Program, NDIS and Mental Health services
  • A commitment to professional excellence, quality improvement and continuous learning
