About the Role

 

Join our team as a Grade 3 Social Worker on a Full-Time time ongoing basis.

 

The Senior Clinician Social Worker is a core part of the Aged Care Services leadership team. This is a unique opportunity to join our large, friendly community-based team. This role will suit an enthusiastic, dynamic and skilled clinician, keen to take responsibility for leading a team ensuring the delivery of safe and high-quality client care, driving service development, and championing impactful continuous improvement activities and research.  

 

This role will work across Transition Care, Aged and Community Care, and the brand new Boollam Boollam Aged Care Centre, based in Heatherton (if required). 

 

In this role, you will:

 

  • Deliver expert clinical care in gerontology, community, and residential aged care settings
  • Provide clinical leadership, supervision and support to Social Workers, Care Partners, Allied Health Assistants and students
  • Lead and participate in aged care reform projects, program transformation, and service planning
  • Coordinate and prioritise caseloads and workflow to ensure effective, safe, and timely client care
  • Undertake comprehensive assessments, care planning, home visits, case management and discharge planning aligned to client goals
  • Contribute to the development of group programs, partnerships with community organisations, and integrated service delivery
  • Actively participate in teaching, mentoring and professional development activities
  • Actively participate and lead  research and continuous improvement initiatives
  • Providing consultation and clinical expertise both within Aged Care Services program and referring partners
  • Coordinate service delivery to maximise operational effectiveness and clinical outcomes.
  • Create and maintain strong partnerships within and outside of Monash Health.

About You

 

You are a dedicated, compassionate social worker with:

 

  • Highly developed clinical knowledge and experience in aged care and gerontology
  • Strong understanding of relevant legislation including the Aged Care Act, Mental Health Act, and other compliance frameworks
  • A proven ability to provide supervision, mentoring, and leadership to others
  • A commitment to delivering holistic, evidence-based, and consumer-centred care
  • An interest in service innovation, quality improvement and multidisciplinary collaboration

 

Education & licence requirements include: 

 

  • Bachelor of Social Work or Masters Social Work Qualifying OR Master of Social Work, OR equivalent as assessed and approved by the Australian Association of Social Workers (AASW)
  • Eligible for ordinary membership of AASW (http://www.aasw.asn.au/)
  • Written evidence of eligibility for AASW membership for overseas qualifications is required.
  • Accredited Social Worker and/or Accredited Mental Health Social Worker: As approved by the AASW
  • Post graduate qualification (or working towards) Leadership/ Management/ Gerontology/ Other relevant field (desirable)
  • Research active (desirable).  
  • Current Victorian driver licence
  • Current and Satisfactory Aged Care Clearance (Police Check/Aged Care Worker Screen)
  • Current and satifactory NDIS Workers Check and NDIS Worker Oritentation Certificate of Completion
  • No current Aged Care Banning Orders in place (as per Register of Banning Orders)
