About The Role
This is an ongoing, part-time (0.6 EFT) opportunity based at Monash Medical Centre.
The Grade 3 Social Worker provides senior clinical care and leadership within the MHAOD Hub services and EMAAP Team, supporting individuals who present to the Clayton Emergency Department with behavioural health concerns and complex psychosocial needs. This specialist role delivers comprehensive psychosocial assessment, therapeutic intervention and short-term assertive outreach (up to 28 days post-discharge) to promote safe, timely discharge and continuity of care.
Working within the ED Hub model, you will collaborate closely with medical, nursing, Emergency Psychiatric Services (EPS) clinicians and peer workers to provide person-centred, recovery-oriented care. The role includes conducting mental state examinations and comprehensive risk assessments, coordinating referrals and community linkages, and reducing psychosocial stressors through collaborative care planning.
As a senior clinician, you will also contribute to service leadership, education and supervision within the program, fostering a strong inter-professional team approach to achieve optimal patient outcomes.
About Us
The MMC Emergency Department has introduced a new inter-professional team which brings together workers from mental health, alcohol and other drug (AOD) services, allied health & lived experience to provide specialised and integrated care to patients presenting to the ED with high acuity mental health, AOD issues, and complex psychosocial needs. This service is called the Mental Health and AOD Hub of which the EMAAP Team operates in, located in the emergency department. The team will provide a seven day a week, extended hours service, as well as an assertive outreach function that supports people following discharge for up to 28 days.
This unique Senior Social Work role will be located within the Clayton Emergency Department as part of the Mental Health and Alcohol and Other Drug (AOD) Hub model of care. The MH&AOD Hub Team is committed to providing a service that acknowledges an individual’s strengths and abilities, and is welcoming, accessible, responsive, collaborative and safe. The team seeks to develop positive relationships with people in crisis, and deliver care and support that respects the individual’s rights, preserves their dignity, and promotes recovery and inclusion.
About You
You are an experienced Grade 3 Social Worker with strong clinical expertise in mental health, alcohol and other drugs, and behavioural health presentations within acute or emergency settings. You bring advanced skills in psychosocial assessment, mental state examination and comprehensive risk assessment, and are confident working with individuals experiencing complex presentations and psychosocial stressors.
You thrive in fast-paced environments and build strong, collaborative relationships with medical, nursing, mental health and peer support colleagues. Your practice is recovery-oriented and person-centred, balancing choice and self-determination with medico-legal requirements and duty of care. You are skilled in short-term, assertive outreach and care coordination, facilitating timely discharge and meaningful community linkages. As a senior clinician, you provide supervision, contribute to clinical leadership, and actively support a collaborative, high-performing team culture.
