About the Role
Join our team as a Grade 3 Social Worker in Adult Mental Health on a full time, ongoing basis, supporting the HOPE team and working from Monash Medical Centre in Clayton.
In the role, you will play a pivotal role in delivering high-quality, person-centred care to individuals engaged with the HOPE program. This position is responsible for providing expert clinical leadership in community mental health, offering care that considers the full spectrum of a person's needs—biological, psychological, and social.
As a key member of a multidisciplinary team, you will undertake comprehensive mental health assessments, delivers evidence-based clinical treatment, and supports individuals experiencing emotional distress. This includes managing a clinical caseload involving risk assessments, crisis management, and care planning—whether in clinic, via outreach, or through telehealth services. In collaboration with consumers, their families, carers, and community agencies, the Senior Clinician develops and maintains individualised treatment and safety plans, ensuring continuity of care and effective discharge planning. The role also involves liaison with internal and external stakeholders to facilitate coordinated service delivery and integration with community supports.
You will lead by example, offering clinical guidance and supervision to staff and students, and contributing to the education and development of the team. This position demands a sound knowledge of mental health legislation, policy frameworks, and service performance indicators. It requires strong interpersonal and collaborative skills, with a focus on recovery-oriented, trauma-informed care that respects individual values, goals, and strengths.
Key responsibilities include:

Conducting comprehensive mental health assessments and registering clients appropriately.

Delivering care aligned with the principles of recovery and the psychosocial model.

Meeting service targets, managing demand, and ensuring service accessibility.

Engaging families and carers in care planning and treatment.

Maintaining compliance with relevant standards, legislation, and safety frameworks.

Liaising with key services such as emergency departments, law enforcement, and community support agencies.

Supporting team development through education, supervision, and professional expertise.
About Us
The (HOPE) initiative provides psychosocial support for people who are identified as having presented with a suicidal or self‐ harm attempt, or suicidal or intentional self‐harm ideation. The initiative aims to provide follow‐up care and support for people who traditionally would not have received such intervention immediately after hospital discharge, with broader referral pathways and targeted psychosocial support. The aim of the initiative is to prevent avoidable suicide attempts and reduce the number of completed suicides by providing targeted psychosocial support services to a cohort of people in an assertive manner in the community for up to 3 months from the time of discharge.

Your proven communication and interpersonal skills enable you to build relationships, manage conflict, influence outcomes, and facilitate open, collaborative discussions. With recent leadership and management experience, you’re confident in driving change, promoting safety, coaching others, and fostering a high-performance culture. You lead with a patient-first approach, value diversity, and bring innovation and evidence-based practice into your work.
