About the Role:

Join our team as a Grade 3 Social Worker, on a part time (0.4 FTE), ongoing basis, working from Monash Medical Centre in Clayton.

 

The key roles of our Supportive Care Social Worker is to provide direct psychosocial support to patients and carers, promote informed choice and facilitate and adapt access to services over time that enhance quality of life and minimise carer burden over the patient's trajectory to end of life care. The incumbent will also play an important team member role in expanding our outreach and deepening our interpretation of supportive care data with a view to continually improve the quality of our service provision. You will be responsible for:

 

  • Supporting other health care professionals, including renal social work colleagues, to develop an applied understanding of the potential benefits of Renal Supportive Care for all patients with CKD 4/5
  • Promoting and facilitate proactive and timely responsive care for patients to optimise well­ being, HRQoL and care in the community
  • Promoting and enable access to timely support systems for carers to ensure their well-being is also considered in the care process.

 

This is a part time, ongoing opportunity working Monday to Friday, 32 hours/ fortnight

 

About Us

 

Monash Health is the largest provider of Nephrology Services in the south east corridor of metropolitan Melbourne and regional Victoria. Services include chronic kidney disease prevention and management, kidney and kidney-pancreas transplantation, a full suite of extracorporeal and peritoneal dialysis kidney replacement therapies, supportive care and comprehensive Nephrology research, education and training programs. Monash Health provides a dialysis service under a hub and spoke (satellite) model with the hub service located at Monash Health Clayton Campus. Dialysis services are designed and located to support a range of patient dialysis needs including inpatient, home and satellite dialysis.  Under the DHHS Vic Renal Capability Framework 2019, Monash Health is a level 6 Hub provider with governance and support system responsibilities for its regional and rural dialysis partners.

 

About You:

You have a passion for providing evidence-based and person-centred care in a multi-disciplinary team and embrace collaboration in care to promote and facilitate positive outcomes, including patient (and carer) choice and life satisfaction, decrease in incidence, prevalence and severity of symptoms, decreased need for presentations to Emergency Departments and hospital admissions, timely referral to palliative care, an increase in end of life care in the patient's place of choice and ultimately a peaceful and dignified death.

 

What you need:

 

  • Bachelor of Social Work or Masters Social Work Qualifying OR Masters of Social Work, OR equivalent as assessed and approved by the Australian Association of Social Workers (AASW)
  • Eligible for ordinary membership of AASW (httR://www.aasw.asn.au/)

 

Written evidence of eligibility for AASW membership for overseas qualifications is required
