About the Role

 

Are you a Social Worker with a passion for delivering timely, evidence-based mental health care? Do you enjoy working collaboratively with consumers to support meaningful change and recovery?

 

The Agile Psychological Medicine (aPM) Clinics provide personalised, flexible, and evidence-based therapeutic care to consumers accessing public mental health services. Working as part of a multidisciplinary team of Psychologists, Social Workers, and Psychiatrists, you will play a key role in supporting consumers experiencing acute mental health challenges and helping them navigate their recovery journey. In this role, you will:

 

  • Conduct comprehensive psychosocial assessments for consumers referred through psychiatric triage, emergency departments, and inpatient services
  • Work collaboratively with consumers to identify their needs and develop recovery-focused treatment plans
  • Provide therapeutic social work interventions and system-based support to improve outcomes and promote recovery
  • Utilise routine outcome measures to inform clinical decision-making and enhance care delivery
  • Contribute to a supportive and innovative multidisciplinary team committed to evidence-based practice and continuous improvement

 

About You 

 

You are a motivated and compassionate Social Worker with experience in mental health and a commitment to delivering high-quality, person-centred care. You will bring:

 

  • Experience working within mental health settings
  • Strong assessment, engagement, and therapeutic intervention skills
  • An interest in systems-based and/or family-inclusive practice
  • The ability to work independently while contributing effectively within a multidisciplinary team
  • A flexible and adaptable approach to managing a dynamic clinical workload
  • A commitment to evidence-based practice and using outcomes to inform care
