About the Complex Care Team
Monash Children’s Complex Care Program is funded by and coordinated in partnership with the Royal Children’s Hospital (RCH) Complex Care Hub. The Monash Children's Complex Care Program supports and coordinates care needs and requirements for families with babies/children with complex medical/ nursing needs who require ongoing medical intervention beyond the hospital admission.
About the Role
The Monash Children’s Complex Care Program-Team Lead is a clinical leadership role overseeing a team of Clinical Nurse Consultants (CNCs) who provide support to complex care patients within the Monash Children Complex care Program. The role provides operational support to the Program’s Medical lead and Operations Manager ensuring strong leadership, effective coordination, and efficient management of our care and service delivery. This role will be working 0.6EFT on a permanent basis.
The Team Leader will be an expert clinical resource and foster strong professional relationship between The RCH Complex Care Hub and Monash Children’s Complex Care Program.
Responsibilities include:
- Coordinate patient and family-centred care in alignment with the RCH Complex Care Model of Care, managing a reduced patient case load within the program.
- Provide clinical supervision and leadership within the Complex care Program
- Mentor and support CNC team through peer collaboration, knowledge sharing, and guidance.
- Liaise and collaborate with the Royal Children’s Complex Care Service and the Family Health Support Team to ensure consistent, standardised service delivery within the model of care.
- Partner with administration staff and the Operations Manager to meet data collection and invoicing requirements within prescribed timelines.
- Work collaboratively as part of a multidisciplinary team to ensure the delivery of comprehensive, high-quality, evidence-based training and support.
About You
- Your attributes and desired experience:
- Previous knowledge and experience working with Paediatric patients with complex medical needs
- Extensive experience working within multidisciplinary teams.
- Paediatric post graduate certificate
- Extensive experience in paediatric nursing.
- Demonstrated ability to lead and manage a specialised team
- Working with Children’s Check
- Experience working within a complex care program desirable (RCH/MCH)
