Our aim is to empower and prepare clients to self-manage their health and health care. Working with clients across the aged spectrum, Monash Health Community provides a continuum of care, supporting clients through all stages of non-acute care from prevention through to rehabilitation. We provide services that support clients in their homes, community based facilities and dedicated Monash Health Community hubs.   

 

About The Role  

We are seeking a passionate, experienced Registered Nurse - Division 1, with excellent communication skills to join the Proactive Management and Support leadership team. The Team Leader performs a key clinical leadership and operational role in supporting the program outcomes and management of staff as directed by the Program Manager, to achieve the goals and directions set out by Monash Health and Department of Health.  

This role is integral to bridging the gap between acute to Community services. Essential to the role is experience within acute as well as an understanding of community services. The role works across various Monash Health sites. We operate from a person - centred care framework and align with the social model of health.  

 

About You 

You are a dynamic clinician with a genuine passion and interest in this unique opportunity who has been developing your leadership skills and is ready to take this next step in your Career. You lead by example, thrive working in a dynamic environment and are truly passionate about delivering a high standard of evidence based care to all clients.   

You have sound skills in assessing and planning patient’s community health discharge requirements and have knowledge of the various community health support services available.  You are an advocate of Monash Health’s Strategic Guiding Principles    

You are also: 

  • committed to a leadership and mentoring role   

  • Committed to delivering patient centred care and advocating for your patients  

  • Enthusiastic, compassionate and able to work collaboratively   

 

What you need 

Essential:   

  • Demonstrated experience with at least 7 years post graduate experience  

  • Current AHPRA registration or registration with National Registration Authority  

  • Valid Driver’s Licence  

  • Australian working rights   

Desirable:   

  • Previous Community experience  

  • Post-graduate qualifications
