Our Journey 

The Aged and Community Care (ACC) Program provides an interdisciplinary service to support older people in the community. The program aims to keep the client independent within their community, proactively promoting health and independence, and self-management whilst assisting with timely interventions to prevent deterioration of the condition, and admission to hospital or residential care. 

We have an exciting opportunity for an enthusiastic Grade 2 Occupational Therapist to take up a fixed term role (0.8-1.0FTE). Also happy to consider Grade 1’s looking for the next step in their career thus strongly encouraged to apply! Please note that whilst position is based in the Cardinia / Casey catchment, you may be required travel to multiple sites across the Monash Health group.

 

A Day in the Life 

As the Grade 2 Occupational Therapist, you will work collaboratively within a multidisciplinary team to support clients to promote independence and to remain living at home and within the community for as long as possible. The role requires provision of evidence-informed assessment, intervention and discharge planning to clients within the Aged and Community Care Program. Occupational Therapists work with clients and/or carers to increase their engagement in meaningful occupation and support healthy living options. Services can take place in a client’s home, in the community and/or by telehealth. You will also be responsible for working within the Aged and Community Care program to provide best practice client care by:  

  • Using specialist skills and knowledge to conduct client assessments and interventions independently, incorporating remedial, compensatory, educational and preventative strategies to achieve optimal evidence-based consumer care  
  • Achieving operational effectiveness whilst maintaining client-centered, compassionate care  
  • Create strong partnerships within and outside Monash Health in collaboration with Senior Clinicians, and Team Leaders.

What you will bring & what you need:  

  • A strong passion to safe best practice & quality care    
  • Commitment to patient-centred care delivery & advocacy   
  • Enthusiasm, compassion & ability to work collaboratively   
  • Bachelor of Occupational Therapy or relevant equivalent degree  
  • Current AHPRA registration  
  • Current Driver’s License

In this role you can look to receive achievable and manageable KPI‘s with a strong focus on delivering evidence based, comprehensive care. You will be supported by a large Occupational Therapy team (within ACC and Monash Health) and have access to senior OT & regular supervision (both 1:1 and peer supervision). You will have opportunities and encouragement to complete quality projects as well as assist with student supervision and support for promotions in the future workforce. All of this can be yours whilst:
