About the Role 

 

Join our dynamic team in this full-time ongoing Grade 1 Social Worker role based at Cranbourne and other Monash Health Community locations.  

 

Work within the interdisciplinary team to provide high quality psychosocial support and advocacy through psychosocial assessment, intervention, counselling, discharge planning, information provision and facilitating access to community services. Social workers function as key members of the team and have a pivotal role where social complexity exists.   

 

Key technical skills specific to this role include:  

 

  • experience working with children and families including at risk children  
  • knowledge and sensitivity to ethical and cultural considerations and how this impacts access 
  • experience in responding to change by contributing constructively to new ideas or change processes 
  • innovative approaches to problem solving for individuals and/or service delivery   

 

About Us  

 

The Children’s Health and Wellbeing Local is an integrated service that provides support, navigation and therapy that considers all aspects of a child’s life from 0-11 years. The service provides wellbeing support, care coordination and therapeutic intervention to help children and families where there are concerns about developmental, behavioural and mental health concerns. The Children’s Local Service is a partnership between community health, mental health, and family support services.  

 

The team works to support priority populations across Greater Dandenong, Casey and Cardinia local government areas with a main site in Cranbourne (Cranbourne Integrated Care Centre) and additional sites across the catchments. 

 

About You  

 

You are seeking a new opportunity to work collaboratively with a passionate interdisciplinary team to deliver a range of specialist medical and allied health services to make a real difference. To ensure your success in this role, you will have the following demonstrated skills, experience and knowledge:   

 

  • A recognised degree in Social Work (Bachelor or Master level), including overseas qualifications assessed by Australian Association of Social Workers (AASW) within the past two years
  • Eligible for membership with Australian Association of Social Workers (AASW)
  • Experience working with children between 0-11 years and their families
  • Understanding of current, relevant theory, practice knowledge and evidence in clinical practice  
  • Ability to work in an interdisciplinary team 
  • Dedicated to safe best practice and quality care   
  • Committed to the delivery of patient centred care and patient advocacy   
  • Current Drivers Licence
  • Prior experience in a healthcare setting (preferred)
