About the Role

Are you a passionate clinician who enjoys bringing people, services and care plans together to make a real difference? An exciting opportunity is available for a Grade 2 Case Manager to join our Community Rehabilitation team at Monash Health, based in Dandenong, on a part time (0.8EFT) ongoing basis.

In this rewarding role, you'll be at the centre of each client's rehabilitation journey, helping people transition from hospital to home and supporting them to achieve their goals, regain independence and reconnect with their community. Working alongside a supportive interdisciplinary team, you'll coordinate care for adults with a diverse range of rehabilitation needs, including neurological, musculoskeletal, cardiac, pulmonary and general rehabilitation.

Your role will include:

  • Completing comprehensive assessments and developing individualised rehabilitation and care plans in partnership with clients and their families.
  • Coordinating care across the interdisciplinary team to ensure a connected, goal-focused rehabilitation experience.
  • Acting as the key point of contact for clients, families, clinicians and external service providers throughout their rehabilitation journey.
  • Supporting safe and successful transitions from hospital to home, including discharge planning and community reintegration.
  • Helping clients and families navigate community support services, including My Aged Care and the NDIS where appropriate.
  • Empowering clients to develop self-management strategies that support long-term independence and wellbeing.
  • Contributing to service improvement, evidence-based practice and a positive, collaborative team environment.

This is a fantastic opportunity for a Grade 2 clinician looking to broaden their experience in community rehabilitation, or an experienced Case Manager seeking a new challenge. You'll have the support of an experienced Senior Clinician Case Manager and Team Leader, while enjoying the variety, autonomy and professional satisfaction that comes with helping people achieve meaningful rehabilitation outcomes.

This position is open to Occupational Therapy, Social Work, Dietetics, Speech Pathology, Physiotherapy, Exercise Physiologist and Nursing disciplines.

About Community Rehabilitation

At Monash Health, we believe rehabilitation is about more than recovery. It's about helping people return to the activities, routines and connections that matter most to them. Our Community Rehabilitation Service provides personalised, goal-focused rehabilitation to adults following illness, injury or a change in their health or function.

Through a combination of home-based, centre-based and telehealth services, our interdisciplinary teams work closely with clients, families and carers to support recovery, build confidence and promote independence. We are a team that values collaboration, innovation and a genuine commitment to person-centred care. We embrace new ideas, support one another and believe that even small achievements can make a big difference in someone's life.

About You

You're a compassionate, organised and proactive clinician who enjoys seeing the bigger picture and bringing people together to achieve great outcomes. You understand that successful rehabilitation isn't just about clinical treatment. It's about listening to what matters to each person, recognising their individual needs and helping them navigate the services and support that will make a lasting difference.

You enjoy the variety of community-based care, thrive in a collaborative team and have the confidence to coordinate complex care while keeping clients at the centre of every decision.

You'll bring:

  • A relevant Allied Health qualification with a minimum of four years' experience, or Registered Nurse qualifications with at least four years' post-graduate experience.
  • Current AHPRA registration and/or eligibility for membership with your relevant professional body, as applicable.
  • Experience in rehabilitation, community health, case management or discharge planning.
  • Strong clinical assessment, care planning and problem-solving skills.
  • Excellent communication and relationship-building skills, with the ability to work collaboratively across disciplines and services.
  • An understanding of community support systems and the importance of coordinated, person-centred care.
  • Strong organisational skills, with the ability to manage competing priorities and a varied caseload.
  • A flexible, positive approach and a genuine passion for helping people achieve their rehabilitation goals.

Why Join Us?

Because helping someone return home, regain their confidence or achieve a goal they once thought was out of reach is what makes this work so rewarding. At Monash Health, you'll be part of a team that recognises the importance of every step in a person's recovery.
