About The Role
Working in line with Monash guideline and procedure, the role of the case manager is to undertake holistic assessment of needs, treatment planning, monitoring, advocacy and linking of the consumer with appropriate community support services, utilising the recovery model principles.
As a clinical role, it will draw on your knowledge and theory in mental health legislation and policy, plus your Occupational Therapy specific skills in psychosocial assessment, an intersectional lens, a holistic approach and discipline specific interventions. Your underpinning interventions will be trauma informed and sensitive, with a purpose to provide excellent clinical person centred care to our community. Comprehensive mental health care means that all aspects of a person’s needs are considered when providing that care.
Demonstrating your expertise in mental health and clinical skills, you will take on a clinical caseload. Your contact with clients will be at our mental health sites, as well as in their own accommodation
Case managers are required to have a good working knowledge of the mental health service structure, including both public health and Community Support services, and the Mental Health and Wellbeing Act 2022. As a clinician you will undertake a clinical caseload including duty, mental state and risk assessment and crisis management.
Dandenong Community Mental Health Team provides an effective and comprehensive service to the Monash Adult Mental Health Program. Working within a multi-disciplinary team we provide treatment and support to individuals experiencing complex mental health problems. We strive to deliver high quality recovery-focused client care. Our aim is to promote self-efficacy and independence, and social inclusion with all the individuals we support.
About Dandenong CCT
The Dandenong Continuing Care Team (DCCT) comprises a multidisciplinary team of case managers including registered psychiatric nurses, occupational therapists, and social workers. The case managers are supported by a consultant psychiatrist, a medical officer or registrar, psychologist and Administration team. The service operates from a community clinic between the hours of 8.30am – 5pm, 5 days per week. Clients are seen both at the clinic and in their own environments. DCCT provides intensive recovery-oriented treatment to consumers experiencing a range of mental health problems. Case managers are required to manage a number of consumers, providing direct support and interventions, coordination of care, and referral to other relevant community professionals, agencies and services to support the individual needs of consumers, their carers and family members.
