About the Role

Join our team as a Clinical Case Manager on a Full-time ongoing basis, Monday to Friday. SECU Diversion operates from Dandenong Community Service Building and Dandenong Hospital. Consumers are seen both at the clinic and in their own environment.  

  • Provide patient centred and recovery focused care to consumers through and outreach, community model of care  

  • Support consumers unique journey to a life inspired by hope and potential in a least restrictive manner  

  • Work collaboratively within a small team comprised of medical, allied and nursing clinicians  

 

About Us

The SECU Diversion Program values and supports each person’s unique journey to a life inspired by hope and potential, in a least restrictive manner. SECU Diversion provides intensive recovery-oriented treatment to consumers experiencing a range of mental health problems. Clinicians are required to manage a number of consumers, providing; direct support and interventions, coordination of care, and referral to other relevant community professionals, agencies and services to support the individual needs of consumers, their carers and family members.  

The service operates from Dandenong Community Service Building between the hours of 08:30-17:00 Monday-Friday. Consumers are seen both at the clinic and in their own environments. Our clinicians are required to have a good working knowledge of the mental health service structure, including both public health and Mental Health Community Support services, and of the Mental Health & Wellbeing Act 2022. 

 

About You

To ensure your success in this role you will bring the following demonstrated skills, experience and knowledge: 

  • Demonstrated capacity to perform community-based and Emergency Department based mental health assessments of people in an acute phase of a psychiatric disorder including comprehensive risk assessments  

  • Demonstrated knowledge, skills and experience in recovery orientated practice, family and carer inclusive practice and supported decision making  

  • Substantial relevant experience in community-based assessment of acute and chronically mentally ill people and of people with a range of mental disorders  

Mandatory requirements of the role:

  • Current authority to practice from AHPRA as a Registered Nurse  

  • Require a Post Graduate Diploma in Mental Health Nursing  

OR  

Completion of a specialist undergraduate Psychiatric Nursing Program  

OR 

Specialist post basic course of training which led to registration as a Division 3 Nurse

  • Work within a small multi-disciplinary team delivering intensive outreach community care to a culturally diverse demographic of consumers 

  • Make a difference in vulnerable consumers lives, supporting them to remain well in the community
