About the Role
The Dandenong Continuing Care Team (DCCT) is seeking a Registered Psychiatric Nurse (Case Manager) full time to join our multidisciplinary team in delivering high-quality, person-centred care to individuals experiencing complex mental health issues.
DCCT is part of the Monash Adult Mental Health Program, providing comprehensive mental health services through a collaborative team approach. Our team consists of case managers, registered psychiatric nurses, occupational therapists, social workers, a consultant psychiatrist, medical officers, psychologists, lived experience workers and an administration team.
We operate from our community clinic, offering flexible care through both in-clinic appointments and home visits, five days a week.
This is a Full-Time Ongoing position.
As a Clinical Case Manager, you will
- Provide direct support and interventions to individuals experiencing mental health problems.
- Undertake holistic needs assessments, treatment planning, and crisis management.
- Coordinate care, liaising with other healthcare professionals and community support services.
- Work closely with consumers, their carers, and families to promote recovery and well-being.
- Demonstrate your expertise through mental state and risk assessments, ensuring comprehensive care and support for each individual.
- Contribute to a recovery-focused approach, supporting consumers' autonomy, social inclusion, and independence.
About you
- Current Authority to Practice from AHPRA as a Registered Nurse.
- Required a Post Graduate Diploma in Mental Health Nursing – or completion of a specialist undergraduate psychiatric nursing program or specialist post basic course of training which led to registration as a Division 3 nurse.
- Post graduate Masters studies or working towards (preferred)
- Dual Diagnosis qualifications (preferred)
- Experience in Alcohol and Other Drugs (preferred)
