Monash Health Aged Care Services is thrilled to announce an exciting opportunity for a highly skilled and experienced Intake Worker to join our Community Support Options team on a full-time basis.

 

About the Role

As a key part of Monash Health Aged Care Services, our Community Support Options team delivers essential services to carers and care recipients across the municipalities of Casey Cardinia, Greater Dandenong, Kingston, and Bayside. This role is integral to our Home Care Package (HCP) program and Commonwealth Home Support Program (CHSP) as we transition to the Support at Home Program under aged care reforms.

In this role, you will:

  • Facilitate access to vital aged care services, enabling clients to live independently in the community.
  • Provide up-to-date service information and assist clients with navigating online tools to access sustainable, appropriate services.
  • Act as a central point of contact for clients, ensuring seamless referral management and support.

What We Are Looking For

We are seeking a dynamic professional with extensive experience in aged care and a passion for making a difference in clients’ lives. To succeed in this role, you will have:

 

Essential Experience:

  • Proven track record in delivering direct support to Home Care Package (HCP) clients as a current or previous HCP Care Manager.
  • Hands-on experience supporting CHSP clients as a current or previous CHSP Coordinator.
  • Expertise in referral management as a central point of access worker in a medium to large healthcare or aged care setting.

Qualifications & Skills:

  • Relevant tertiary qualifications in health or social sciences.
  • In-depth understanding of aged care reforms and client-directed care models.
  • Strong computer literacy and proficiency in online service tools.
  • Excellent communication, organisational, and interpersonal skills.
  • Ability to work autonomously while thriving in a collaborative team environment.
