About the Role
The role is a 12 month maternity leave backfill and is for a Registered Psychiatric Nurse to work in multi-disciplinary team comprising of Social Workers, Psychiatric Nurses, a Psychologist, Exercise Physiologists, an Aboriginal Clinician and a Community Development Worker. The role is a clinical, outpatient position working 8:30am - 5pm (Tuesdays 11:30am - 8pm) in an appointment -based, non-emergency service.
The key responsibilities of the role are to:
- Undertake comprehensive mental state, psychosocial, functional and physical assessment of participants within a community setting, and in conjunction with relevant others
- Formulate, implement and evaluate individual service plans for participants with appropriate follow-up mental health counselling care including referral to other agencies as required
- Undertake initial and ongoing risk assessments of participants and develop risk management plans to provide a safe environment for the participants and others
- Liaise with internal and external health care providers and other agencies

The Youth and Family Program is comprised of numerous disciplines that focus on the assessment and delivery of youth and family focused care. The multi/inter disciplinary team is committed to working with vulnerable and marginalised young people and adults by providing high quality, multidisciplinary programs which embrace client centred, trauma informed practice and evidence-based services
Forensic Mental Health in Community Health
Forensic Mental Health in Community Health (FMHiCH) is required to provide an efficient, high quality and comprehensive service to adults aged 18 years and over on Corrections orders or parole residing in the City Of Greater Dandenong, Casey and Cardinia areas. The FMHiCH Program is a comprehensive mental health and broader health evidence-informed service. The aim of the FMHICH Program is to assist eligible individuals to access the mental health care they need and enable them to satisfy the requirement of their Mental Health and Treatment Rehabilitation (MHTR) condition while reducing their recidivism risk. It is intended that this Program will assist with providing timely and appropriate assessment and treatment to those who require it, and reduce risk to the community.
