Our Journey
The Residential Aged Care Liaison (RACL) works closely within the social work department at Monash Health. We are currently advertising for a permanent Residential Aged Care Liaison role within the Social Work Department at Dandenong Hospital, Monash Health. This is a 0.8 EFT permanent part time role and would be suitable for a Registered Nurse OR Grade 2 Social Worker.
A Day in the Life
The Residential Aged Care Liaison (RACL) works closely with patients, carers and families who have been assessed as requiring residential aged care/supported accommodation and/or Transitional Care, in order to enable transfer to the appropriate discharge destination, such as residential aged care. The role works with older people and with younger disabled people who meet criteria for residential aged care.
The incumbent works closely with the Social Work, Allied Health and Multidisciplinary team and key stakeholders such as the Aged and Rehabilitation Access Service. The role requires knowledge of the commonwealth funded aged care system and state funded organisations i.e., Supported Residential Services and the service sector in order to provide advice and support for patients, families, carers and the broader health care team, including close liaison with the Social Work team.
The Social Work Service at Monash Health assists patients and families with psychosocial support and advocacy through assessment, counselling, information provision, access to community resources, and discharge planning. Social Workers function as members of the multidisciplinary team and have a pivotal role where social complexity exists.

You will be a member of a dynamic and supportive Social Work and Allied Health team working in a collaborative model of care across Monash Health. The position works closely with the internal and external stakeholders to provide care founded on current evidence-based practice for patients accessing care through the inpatient setting. You will have demonstrated experience working collaboratively with the broader multidisciplinary team and possess excellent written and verbal communication skills.
What you need:

Bachelor of Social Work or Masters Social Work Qualifying OR Master of Social Work, OR equivalent as assessed and approved by the Australian Association of Social Workers (AASW)

Eligible for ordinary membership of AASW if overseas trained (http://www.aasw.asn.au/)

Registration with Nursing and Midwifery Board of Australia (NMBA) via the Australian Health Practitioner Regulation Agency (AHPRA), and consent to ongoing registration checks via Monash Health’s electronic credentialing systems

Current and ongoing employment screening, including but not limited to, National Police Check, employee Working with Children Check (WWC) and where relevant Aged Care Worker Screening, and as required National Disability Insurance Scheme (NDIS) Worker Screening

Recent and relevant experience demonstrating the appropriate competencies and skills (May include a required minimum number of years’ experience)

Current Victorian Drivers Licence
