Expressions of Interest are invited from a senior clinician within Monash Health looking to expand their skill set by backfilling a temporary grade 4/3 position.

 

The position covers 16 weeks of leave and involves the provision of care co-ordination and support for mental health program and consumers presenting with personality disorders and/or those identified as frequent presenters to our emergency departments.

 

The role involves travel across Monash Health sites in a fleet car and is otherwise based at Cleeland Street, Dandenong. Days of work are flexible except for Monday which is a required day.

 

The Senior Care Co-ordinator is a flexible, responsive role that provides support to the Adult Mental Health Program to implement clinical pathways and best practice approaches in working with consumers with personality disorders and those identified as frequent presenters to our emergency departments. 

 

The role also involves providing supportive contact to a small number of identified consumers who are not connected to other parts of the Adult Mental Health Program.

 

Required qualifications/skills and competencies include:

  • A flexible, responsive and active attitude
  • Good knowledge and understanding of the Adult Mental Health Program structure & functions
  • Competence in understanding mental health issues especially personality disorders
  • Initiative and drive
  • Ability to effectively communicate with individuals, teams and diverse stakeholders
  • Adhere to professional guidelines/codes, organisational policies/procedures and legislative requirements
  • Good time management
