About the Role

 

The focus of HITH is to optimise patient outcomes by providing intensive treatment and support in a least restrictive model to ensure that community treatment is optimal and focuses on recovery principles of care.

 

The position will require an individual with proven communication skills which will enable them to liaise closely with a number of community agencies and clinical teams.

Clinical requirements:

  • Ensure the delivery of a high standard of evidence-based nursing care to consumers
  • Understands the interrelationship between mental health and physical co-morbidities
  • Demonstrates an understanding of common screening tools used in mental health assessment and applies them in clinical practice.
  • Provides evidence-based interventions and support for people with mental health issues.

This a fulltime position open to Grade 4 Registered Mental Health Nurses and operates Monday to Friday 9am-5pm

 

About Dandenong ACIS

 

The Dandenong ACIS includes Emergency Psychiatric Services (EPS), Crisis Assessment and Treatment Team (CATT) Hospital in the Home (HITH), Services are provided 24 hours a day, seven days a week. The Service provides timely comprehensive assessment and deliver evidence-based treatment in the most appropriate setting at the ‘front end’ of service delivery. This includes short-term interventions and treatment for those whose illness can be readily managed, as well as seamless engagement with longer-term care for those with relapsing or enduring conditions.

 

The priority is to help a person to remain in or return to their local community as quickly as possible, if assessed as clinically safe and appropriate given the circumstances. Interventions are tailored to individual consumer and family needs as well as circumstance 

 

About HITH

 

Within a HITH model of care, the clinician will assess all patients referred to HITH; to plan, implement, coordinate formulate and review treatment in conjunction with the HITH Consultant Psychiatrist.

The focus of HITH is to optimise patient outcomes by providing intensive treatment and support in a least restrictive model to ensure that community treatment is optimal and focuses on recovery principles of care.

 

The position will require an individual with proven communication skills which will enable them to liaise closely with a number of community agencies and clinical teams including the Emergency Department, Victoria Police, General Practitioners to ensure best practice care is provided. Within a multidisciplinary team structure, the HITH provides timely and holistic care to our consumer group presenting in crisis. This incorporates comprehensive assessment and delivery of evidence-based treatment and onward referral.
