About the Role
Join our team as a Grade 2 Social Worker on a part time (0.8 FTE), fixed term basis until May 2026. This role is based within the Youth & Family program at Thomas Street, Dandenong however you may be required for work at any of the Monash Health sites.
In this dynamic and rewarding role, you will deliver high-quality clinical care as part of a multidisciplinary team, providing individual assessments, therapeutic interventions (both individual and group), and care coordination tailored to the unique needs of young people.
You will manage an active caseload and play a key role in enhancing outcomes through collaboration with internal and external service providers. This position also offers the opportunity to contribute to the development of future practitioners by supervising social work students on placement.
As part of a supportive team, you’ll contribute to the seamless coordination of care, demonstrate clinical leadership, and engage in ongoing professional development. You’ll also be instrumental in embedding and shaping a new and evolving program that addresses the complex needs of young people in our community.
Please note this service includes after-hours work on Tuesday 9:00am-17:30pm and Thursday 10:30am-19:00pm.
About Us
The Youth and Family Program is comprised of numerous disciplines that focus on the assessment and delivery of youth and family focused care. The multi/inter disciplinary team is committed to working with vulnerable and marginalised young people and adults by providing high quality, multidisciplinary programs which embrace client centred, trauma informed practice and evidence-based services.The Youth and Family Team supports young people 15-25 years of age (or equivalent developmental age) at our Tuesday and Thursday after school hours Youth Health Hubs, by providing individual assessments, support services and group programs.
About You
You are a qualified Social Worker with at least two years of direct practice experience, including working with young people adjusting to complex health conditions and mental health challenges. You have a strong understanding of the unique needs of vulnerable youth, including those from refugee and culturally and linguistically diverse (CALD) backgrounds.
You bring well-developed skills in psychosocial assessment, therapeutic intervention, and care coordination, with the ability to assess and manage high-risk behaviours. You have experience facilitating group programs and have contributed to the design and delivery of targeted clinical interventions for at-risk young people.
Your practice is grounded in relevant social work theories, a sound understanding of legal and ethical frameworks, and a demonstrated ability to navigate complex service systems such as Child Protection, Family Violence, and the NDIS. You work collaboratively in multidisciplinary settings, demonstrate emerging leadership, and are committed to continuous learning, quality improvement, and supporting the next generation of practitioners through student supervision and professional development initiatives.
Qualificiation and licence requirements of the role include:

Bachelor of Social Work or Masters Social Work Qualifying OR Masters of Social Work, OR equivalent as assessed and approved by the Australian Association of Social Workers (AASW)

Eligible for ordinary membership of AASW (http://www.aasw.asn.au/)

Written evidence of eligibility for AASW membership for overseas qualifications is required.
