About the Role
Join our Aged & Community Care Profram as a Grade 2 Social Worker on a part time (0.5 EFT) ongoing basis. This role will see you working across multiple Monash Health sites, with a base at Dandenong Hospital.
In this role, you’ll provide comprehensive assessment, care planning, intervention, and discharge planning for clients within the community setting — supporting them in their homes, community health centres, and through group programs. You’ll be part of a dynamic and collaborative multidisciplinary team, contributing your expertise to ensure best practice care while also supporting service development and quality initiatives.
As a Grade 2 clinician, you’ll also have opportunities to mentor and supervise students, participate in research and quality improvement projects, and share knowledge within the social work team and broader allied health network. This position offers excellent opportunities to develop your clinical and leadership skills, with ongoing support from a Grade 3 Social Worker and access to in-house professional development within a supportive and experienced team.
About Us
The Aged and Community Care (ACC) Program within Monash Health Community consist of a multi-disciplinary team providing an interdisciplinary service to support older people within their own environment. The program aims to keep the client independent within their community by proactively promoting health, independence, and self-management. Our team of highly experienced Physiotherapists work to provide timely, person-centred interventions to improve quality of life and prevent deterioration of health.
