About the Role
An exciting opportunity exists for an experienced Grade 2 Social Worker to join the Social Work Service at Monash Health in a full-time, ongoing role based at Dandenong Hospital.
As part of a highly collaborative multidisciplinary team, you will provide high-quality psychosocial assessment, intervention, counselling, advocacy and discharge planning for patients and families experiencing complex health and social needs. The role carries a designated clinical caseload and requires a strong understanding of evidence-informed practice and contemporary healthcare systems.
You will work within the adult acute team and play a key role in supporting safe patient care and positive health outcomes. The position also provides opportunities to contribute to quality improvement, service development, supervision, teaching and research initiatives within the broader Social Work Service.
A sound working knowledge of Family Violence, Child Protection, My Aged Care, Transition Care Program, NDIS, Mental Health and recovery-oriented practice is essential. Practice is aligned with the professional standards and ethical framework of the Australian Association of Social Workers (AASW).

You are a motivated and experienced Social Worker who demonstrates strong clinical reasoning skills and a commitment to patient-centred care. You work collaboratively within multidisciplinary teams and bring excellent communication and stakeholder engagement skills.
