Monash Health Mental Health Services provide comprehensive, integrated care across community, inpatient, and specialist settings, supporting a diverse population of nearly one million people across Melbourne’s southeast.

 

The Adult Inpatient Unit (P Block) is part of a broader, highly regarded Mental Health Program. Working within this program, you’ll be part of a strong professional and clinical governance structure that prioritises innovation, collaboration, and high-quality consumer care.

 

About the Role

 

Join Monash Health’s Adult Mental Health Inpatient Unit (P Block) at Clayton as a Social Worker (Grade 3). This is a permanent full-time position, working Monday to Friday, within a 25-bed acute inpatient unit supporting adults aged 25–64 experiencing acute mental health challenges.

 

In this role, you will deliver high-quality, evidence-based social work services as part of a multidisciplinary team. You will maintain an active caseload and provide comprehensive bio-psychosocial assessments, crisis intervention, and discharge planning to support recovery and continuity of care.

 

You’ll play a key role in:

 

  • Supporting consumers, families, and carers through complex mental health presentations
  • Addressing barriers to discharge and coordinating access to community, housing, and support services 
  • Facilitating group programs and delivering psychoeducational sessions 
  • Collaborating closely with inpatient teams, community clinics, PARCS, and external stakeholders to ensure seamless transitions of care 

 

You will be supported by a Grade 4 Senior Social Worker and contribute to supervision, student support, and continuous service improvement initiatives.

 

About You

 

You are a passionate and adaptable Social Worker with a strong commitment to delivering person-centred care in a fast-paced clinical environment. You thrive in multidisciplinary settings and bring a proactive, solutions-focused approach to complex presentations.

 

You will demonstrate:

 

  • Experience (or strong interest) in mental health, acute care, or hospital settings
  • Proven ability to conduct psychosocial assessments and deliver evidence-based interventions 
  • Strong understanding of the Mental Health & Wellbeing Act, discharge planning, risk assessment, and community service systems 
  • Confidence in managing sensitive and complex situations, including family violence, homelessness, and vulnerable populations 
  • Excellent communication and stakeholder engagement skills across clinical and external services
