About Us & the Role
An exciting opportunity exists for an experienced and passionate Grade 3 or Grade 4 Social Worker to join the Psychiatric Triage Service (PTS) on a full-time, ongoing basis.
Whether you're an experienced Grade 3 clinician looking to further develop your expertise, or an established Grade 4 clinician seeking to provide advanced clinical leadership, this role offers the opportunity to make a meaningful impact while continuing to grow your career within a highly respected specialist mental health service.
The PTS is a 24-hour, seven-day telephone-based assessment and response service, providing a critical entry point to Monash Health Mental Health Services. The service supports adults aged 18–64 years and provides after-hours mental health assessment and advice for young people under 18 and older adults aged 65 years and over. Working within a highly skilled multidisciplinary team, you'll provide timely mental health triage, assessment and care coordination for consumers experiencing acute mental health concerns.

Every day you'll make critical clinical decisions that shape a person's mental health journey. Working at the front door of Monash Health Mental Health Services, you'll assess complexity, manage risk, collaborate with specialist teams and ensure consumers receive the right care at the right time. It's a role that offers exceptional variety, develops advanced clinical judgement and provides opportunities for leadership, influence and ongoing professional growth. Your role will include:
- Conducting comprehensive preliminary mental health and risk assessments via telephone.
- Determining the nature, urgency and level of mental health concerns.
- Coordinating timely and appropriate responses through referral to specialist mental health services and external agencies.
- Providing expert clinical advice, consultation and risk management support to consumers, carers and referring professionals.
- Working collaboratively with internal and external stakeholders to ensure safe, person-centred and recovery-oriented care.
Grade 4 clinicians will also have opportunities to provide advanced clinical leadership, mentor and support colleagues, contribute to service improvement initiatives, and promote evidence-informed practice across the service.
About You
You are an experienced Mental Health Social Worker who thrives in a dynamic and fast-paced environment. You bring excellent clinical judgement, strong assessment skills and a commitment to delivering high-quality, recovery-oriented care. We're looking for clinicians who demonstrate:
- Experience assessing and managing consumers experiencing acute and complex mental health presentations
- Sound knowledge of the Victorian Mental Health and Wellbeing Act, relevant legislation and contemporary mental health practice
- Excellent communication, clinical reasoning and decision-making skills
- The ability to confidently assess and manage risk while working collaboratively with consumers, carers and multidisciplinary teams
- A commitment to recovery-oriented, trauma-informed and person-centred practice
- The ability to work autonomously while contributing positively to a supportive team culture
For Grade 4 applicants, experience providing clinical leadership, supervision, mentoring, quality improvement or service development will be highly regarded.
