Aged Care Clinical Assessor (Aged Care Assessor)

About the role

As an Aged Care Assessor, you will work closely with seniors and their families to assess their care needs and help them access appropriate services and resources. Your role will directly impact the wellbeing of older people, supporting them to live with dignity, safety, and comfort.

Key responsibilities

• • Conduct thorough assessments of older people to determine care needs, preferences, and eligibility for aged care services.
  • Work with clients, families, and care providers to develop personalised care plans that support physical, emotional, and social needs.
  • Complete assessments in line with current legislation, policies, and quality standards.
  • Collaborate with health professionals, government agencies, and service providers to coordinate services and support holistic care.
  • Advocate for the rights of older adults and ensure their voices are heard throughout the assessment and care process.
  • Monitor and review care plans to ensure they remain relevant and effective as client needs change.

About you

We are seeking individuals who are passionate about aged care and public health. To be successful in this role, you will bring:

• • Experience and/or qualifications in Aged Care, Nursing, Social Work, or a related field.
  • Appropriate qualifications.
  • Proven clinical aged care needs assessment experience.
  • Strong communication, empathy, and assessment skills.
  • Knowledge of aged care services, policies, and regulations.
  • A commitment to delivering person-centred care and advocating for older adults’ rights.
  • Ability to work independently and as part of a multidisciplinary team.
