Welcome to Monash Health Residential Aged Care Services
Monash Health recently undertook a revamp of its leisure and lifestyle program that is delivered across all Monash Health Residential Aged Care and the Transition Care Program.
This new program supports a seamless transition into the new centre, ensures equity in service delivery, and enhances the resident experience across all Monash Health residential aged care services sites.
Due to commence in mid September 2025, we are currently recruiting for Grade 2 Leisure and Lifestyle Assistants. There are ongoing part time or full-time positions based across the new Boollam Boollam Aged Care Centre, Chestnut Gardens Nursing Home and the Transition Care Program.
The new Boollam Boollam Aged Care Centre boasts state of the art facilities purposely built for our residents to ensure they receive the very best wellbeing activities through our lifestyle program. Amenities at the new centre include:

About the role:
We are currently seeking compassionate and motivated Leisure and Lifestyle Assistants to join our Residential Aged Care Services team.
The Leisure and Lifestyle Assistant works with residents/clients, families, other care team members and volunteers to enhance the quality of residents’ lives by engaging residents in meaningful activities that are satisfying to residents and promote their emotional, spiritual and psychological wellbeing.
The Leisure and Lifestyle Assistant supports residents to realise their potential, achieve goals and experience purpose and fulfilment.
In this role, you will;

Plan and deliver individual and group activities that reflect residents’ interests, preferences, and goals

Work collaboratively with residents, families, care staff, and volunteers to enhance resident satisfaction and wellbeing

Contribute to personalised wellbeing care plans and ensure timely documentation and evaluation
-
Support the coordination and delivery of monthly activity calendars, special events, and community outings
-
Provide emotional and spiritual support, especially during end-of-life care
-
Maintain safe, inclusive, and welcoming activity environments

Uphold Monash Health iCARE values and adhere to infection control, OHS, and Aged Care Standards including SIRS and NDIS requirements
About You:
You are passionate about enriching the lives of older people, empathetic, reliable, a strong communicator, and committed to delivering person-centred care. You can work both independently and as part of a team. This is your opportunity to make a real difference by creating meaningful moments for our residents every day.
You also possess;

Valid NDIS Clearance and completion of the NDIS Worker Orientation Module (essential)

Valid police check clearance including a renewal every 3 years (essential)

Certificate IV in Leisure and Health (CHC43415) or equivalent qualification (essential)

Current Victorian Driver’s Licence (essential)

Ability to work collaboratively within a team and follow direction as needed

Self-aware and committed to reflective practice and continuous improvement

Strong commitment to providing resident-centred care
-
Effective time management skills to meet key outcomes

Well-developed verbal, written, and computer communication skills
