Help shape a new aged care triage service supporting older people across the Vic 2 Region.
About the Role
An exciting opportunity is available for an experienced clinician to join Monash Health’s Aged Care Assessment Service (ACAS) as a Senior Clinician, as we lead delivery of the Vic 2 Region Single Assessment Service (SAS).
This is an opportunity to join at the beginning of something new, the establishment of a new triage service. We are growing our workforce, creating a new multidisciplinary team focused on making aged care assessment simpler, more accessible and more consistent for older people. As Senior Clinician, you’ll provide clinical and operational leadership across triage, assessments and reviews, supporting our multidisciplinary team to deliver high-quality, consistent care.
Your role will include:
- Providing senior clinical guidance and supporting complex decision-making.
- Supporting the establishment and development of the new triage service.
- Providing supervision, mentoring and professional support to clinicians.
- Supporting orientation, education and capability development.
- Leading quality improvement and service development initiatives.
- Building strong partnerships across health, community and aged care services.
- Supporting person-centred practice focused on dignity, choice, independence and reablement.
You may also maintain a clinical caseload where complex care needs, joint assessments or senior clinical expertise are required. Flexible and remote working opportunities are available, alongside the operational requirements of the service.
This position is open to Social Work, Occupational Therapy, Physiotherapy & Nursing disciplines.
About You
You’re an experienced clinician with strong clinical judgement and a genuine interest in developing people and improving services. You’ll enjoy working in an evolving environment where your ideas, experience and leadership can help shape how a new service operates. You will bring:
- Significant experience in aged care, community health, rehabilitation, gerontology and/or complex care.
- Strong assessment, clinical reasoning and care-planning capabilities.
- Experience providing clinical leadership, supervision or mentoring.
- An understanding of person-centred care, wellness and enablement.
- Strong communication, collaboration and stakeholder engagement skills.
