About the Role
The Social Work Department is looking for a Grade 1 fulltime permanent Social Worker to join our growing team at Monash Health, Kingston Centre, working in the subacute hospital setting. You will have the opportunity to join a dynamic social work and multi-disciplinary team where you will have a pivotal role in contributing to a positive team culture.
As a Grade 1 Social Worker you will be provided with an opportunity to be challenged plus demonstrate your skills including:

Working with people from a range of diverse backgrounds

Complete complex psychosocial assessments,
-
Development of patient centred care plans and complex discharges.
You will be a member of a dynamic and supportive Social Work and Allied Health team working in a collaborative model of care across Monash Health. The position works closely with the internal and external stakeholders to provide care founded on current evidence-based practice for patients accessing care through the inpatient and community based setting.

You will also require:

Bachelor Social Work or Master Social Work (Qualifying)

Current driver’s licence

Eligible for AASW membership including continuing professional education
