About the Role

 

A unique opportunity exists for talented Social Workers to join the Monash Health Community Rehabilitation program (Cranbourne & Pakenham) in fixed term roles. Community Rehabilitation is seeking Grade 2 Social Workers for multiple leave cover roles across Community Rehabilitation sites from December 2025 through to June 2026. These roles present an excellent opportunity to broaden your skills and gain experience in the Community setting, and will be well suited to an experienced Grade 1 or a Grade 2 Social Worker looking for a change. We welcome applications for part of or all of the leave cover period. This role affords the opportunity to work in a full time or part time in the hours that suit you in the week. 

 

The Community Rehabilitation Social Worker provides high quality, short term Social Work service to clients and their families and/or carers, in line with professional standards and organisational guidelines. You will be part of a large multidisciplinary team who foster a culture based on support, inclusivity, and a passion for what we do.

Community Rehabilitation has sites at Kingston, Dandenong, Springvale, Cranbourne and Pakenham. We promise you that no two days will be the same. This role will see you treat a wide range of patients with varying clinical presentations across neurological, cardiac, pulmonary and oncology streams. About You

  • Be truly passionate about delivering a high standard of evidence-based care to all clients    

  • Have enthusiasm and compassion for the clients in your care   

  • Current Working with Children’s Check  
  • Current registration (or eligible for registration) with the Australian Association of Social Workers 

  • Current Drivers Licence
