About the Role
We are seeking a passionate and experienced Social Worker to join the PGNH (Psychogeriatric Nursing Home) In-Reach Team, part of the Older Adults Mental Health Service at Monash Health. This unique opportunity involves delivering client-focused, multidisciplinary care to older adults (generally aged 65+) with mental illness or Behavioural and Psychological Symptoms of Dementia residing in Monash-run Residential Aged Care Facilities.
This is a full time, fixed term position until May 2026 and will be based at The Kingston Centre in Heatherton.
As a key member of our team, you will be involved in:
- Conducting comprehensive assessments and treatments
- Developing behaviour management plans
- Providing clinical education to carers and staff
- Advocating for and representing consumers at mental health tribunals
- Supporting clients and families through therapeutic and systems-based interventions
- Building collaborative partnerships with community organisations and external agencies

- Community Teams: 7 community teams across Kingston Centre and Endeavour Hills
- Inpatient Units: Biala (Kingston Centre) and Unit 3 (Dandenong Hospital)
- Residential Services Partnerships: Boollam Boollam Aged Care Centre (BBACC), Chestnut Gardens
The PGNH In-Reach Team is a critical part of our community service, delivering targeted mental health interventions and education in RACFs. Our multidisciplinary team includes Psychiatrists, Medical Registrars, Social Workers, Occupational Therapists, Neuropsychologists, and Nursing Educators. About You
You are a values-driven Social Worker with a strong interest in aged mental health and a commitment to quality care. You’re ready to thrive in a collaborative environment where your voice matters and your expertise is valued. Your approach is compassionate, recovery-oriented, and culturally sensitive.
You have demonstrated:
- Ability to provide psychosocial assessments and therapeutic support
- Confidence in MDT settings and interdisciplinary case planning
- Experience working with aged clients, dementia, and complex mental health presentations
- Skills in advocacy and navigating legal processes (e.g. VCAT, Mental Health Tribunal)
- A strong commitment to client safety and continuous quality improvement
- Ability to build strong partnerships with families, carers, and community agencies
Education & Registration requirements include:
- Bachelor of Social Work or Master of Social Work (Qualifying) or equivalent as assessed by Australian Association of Social Workers (AASW)
- Eligibility for AASW membership (evidence required for overseas qualifications)
- Accredited Social Worker and/or Accredited Mental Health Social Worker (highly regarded)
