About the Role 
 
We currently have a vacancy for a Grade 2 Social Worker on a full time, ongoing basis to work at Monash Health, Kingston Centre, working in the subacute hospital setting.
 
As a Grade 2 Social Worker, you will manage a diverse clinical caseload, providing comprehensive assessment, counselling, crisis intervention, advocacy, and discharge planning. This includes supporting vulnerable populations, such as individuals impacted by family violence, elder abuse, and child protection concerns, while working within relevant legislative frameworks. You will also coordinate access to essential services including My Aged Care, the National Disability Insurance Scheme (NDIS), and community-based supports to facilitate safe and effective transitions of care.
 
You will be a member of a dynamic and supportive Social Work and Allied Health team working in a collaborative model of care across Monash Health. The position works closely with the internal and external stakeholders to provide care founded on current evidence-based practice for patients accessing care through the inpatient and community based setting. 
 
About Us
 
The Social Work Service at Monash Health assists patients and families with psychosocial support and advocacy through assessment, counselling, information provision, access to community resources, and discharge planning. Social Workers function as members of the multidisciplinary team and have a pivotal role where social complexity exists.
 
The Social Work bed-based service consists of over 90 staff providing a service at the Kingston Centre, Moorabbin Hospital, Monash Medical Centre, Dandenong Hospital, Casey Hospital and the Victorian Heart Hospital.
 
The Social Work Service operating hours are standard business hours Monday to Friday. A weekend and on call service is provided by roster at all acute sites.
 
 
About You 
 
You are a motivated and adaptable Social Worker with strong clinical skills in assessment, counselling, crisis intervention, and discharge planning. You bring experience working with vulnerable populations and have a sound understanding of key systems such as family violence, child protection, My Aged Care, NDIS, and mental health frameworks.
 
A collaborative team player, you communicate effectively, build strong stakeholder relationships, and are committed to delivering patient-centred, evidence-based care. You thrive in fast-paced environments, use sound clinical judgement, and approach challenges with flexibility. You are open to working across different clinical areas and sites, participating in on-call as required, and are committed to ongoing professional development.
 
You will have demonstrated experience as a social worker with relevant qualifications (Bachelor Social Work or Master Social Work (Qualifying)) & eligible for AASW membership including continuing professional education. You will also bring with you an experience working in a community health or hospital setting and a current driver’s licence.
