About the Role
Social Worker or Occupational Therpaist | Part Time Ongoing - 0.6 EFT | Kingston Centre
An exciting opportunity is available for a Team Leader – PGNH In-Reach Team to join our Older Adult Mental Health Service on an part time, ongoing basis (0.6 EFT).
In this senior clinical leadership role, you will support older adults living in Residential Aged Care Facilities (RACFs) who experience complex mental health needs and/or Behavioural and Psychological Symptoms of Dementia (BPSD). Working within a multidisciplinary team, you will provide advanced clinical guidance, coordinate service delivery, and ensure responsive, person-centred care for some of our most vulnerable consumers.
As Team Leader, you will play a key role in supporting clinical decision-making, overseeing workflow and service coordination, and fostering a collaborative team culture. You will also provide leadership, mentoring, and education to clinicians and key stakeholders, helping to promote best-practice approaches to care across residential aged care settings. In this role, you will:
- Provide advanced clinical leadership within a specialist Older Adult Mental Health service
- Support complex assessments, risk management, and care planning
- Lead multidisciplinary discussions and coordinate service delivery
- Mentor and develop clinicians while promoting evidence-informed practice
- Build strong partnerships with Residential Aged Care Facilities and key stakeholders
- Lead local operational meetings and contribute to program-wide initiatives
- Drive quality improvement, clinical governance, and service development activities
- Support the implementation of innovative and contemporary models of care
This opportunity is open to Social Work & Occupational Therapy disciplines and is ideal for an experienced mental health clinician who is passionate about leadership, collaboration, and improving outcomes for older adults living in residential aged care.
About You
You are a highly skilled and experienced mental health clinician with a passion for leadership, collaboration, and improving outcomes for older adults. You bring advanced clinical knowledge, sound judgement, and the ability to confidently navigate complex presentations and service environments. You are an engaging and supportive leader who values multidisciplinary teamwork and fosters a positive, accountable, and learning-focused culture.
You will also demonstrate:
- Strong leadership and team development capability
- Excellent communication and stakeholder engagement skills
- Sound understanding of clinical governance and risk management
- The ability to lead change and support service improvement initiatives
- Strong organisational and problem-solving skills
- Confidence working collaboratively across multidisciplinary and interagency settings
Qualification & Licence requirements include:
- Relevant qualification in Social Work and eligibility for membership with the Australian Association of Social Workers (AASW) or Current general registration with the AHPRA – Occupational Therapy, with no restrictions.
- Minimum 5 years’ experience in mental health
- Experience in Older Adult Mental Health strongly desirable
- Understanding of aged care and relevant legislation highly desirable
- Sound knowledge and understanding of the Mental Health and Wellbeing Act 2022 and other relevant legislation
- Current Victorian Driver Licence and access to a reliable vehicle
