About the Role:

Join our team as a Leisure and Lifestyle Assistant on a Part Time (2 days per week Fri/Sat), fixed term (July 2025), at Yarraman Nursing Home, Noble Park

The Leisure and Lifestyle Assistant plays a key role in improving the quality of life for residents by collaborating with residents, their families, the care team, and volunteers to create engaging and meaningful activities. These activities are designed to promote emotional, spiritual, and psychological wellbeing while ensuring residents find purpose, achieve goals, and experience fulfillment. Working under the guidance of the Facility Manager, the Leisure and Lifestyle Assistant is responsible for planning and delivering a range of individual and group activities, tailored to meet residents’ preferences and needs. The role involves evaluating the effectiveness of these activities to ensure the best outcomes for each resident. Additionally, these activities encourage social connections, foster friendships, and help build a sense of community both within the facility and beyond.

Yarraman Nursing Home is located in suburban Noble Park and provides high level care and assistance for 30 residents who can no longer live independently. About You:

We are looking for a compassionate and dedicated individual who is committed to enhancing residents' wellbeing through engaging and purposeful activities. You will work closely with residents, families, and the multidisciplinary care team to ensure that each resident’s needs are met with respect and empathy, while promoting their cognitive, social, and emotional wellbeing. You will play a key role in planning and delivering activities, ensuring a safe and supportive environment, and providing spiritual and emotional support, especially during end-of-life care.

You will also:

  • Complete residents' “My Wellbeing Profile” (MAR-16) in collaboration with residents and their families within the required timeframe, ensuring regular evaluations are conducted and communicated to the care team.
  • Develop tailored wellbeing care plans for each resident based on their individual needs and preferences, incorporating both individual and group activities to enhance cognitive and emotional wellbeing.
  • Plan wellbeing activities and outings in partnership with residents and families, ensuring they are included in a monthly activity calendar.
  • Provide a weekly/monthly activity program calendar to staff and residents at least two weeks in advance of the planned activities.
  • Prepare and set up activities, ensuring all equipment meets infection prevention and OHS standards, and clean up after events.
  • Ensure the environment is safe and conducive to the wellbeing of residents, staff, and visitors.
  • Assist residents in accessing community services, acting as an advocate for their needs.
  • Collaborate with residents, families, the care team, and volunteers to achieve quality care outcomes and resident satisfaction.
  • Participate in managing residents’ responsive behaviours in accordance with their individual behaviour management plans.
  • Provide emotional and spiritual support to residents and families, especially during the end-of-life phase.

 

Mandatory requirements of the role - qualifications, licences, memberships,

· CHC43415 - Certificate IV in Leisure and Health or equivalent
