About the Role
As the Continuing Care Team and Mobile Support and Treatment Service, Team Manager, you are expected to provide both clinical, organisational and operational oversight to the Continuing Care Team and Mobile Support and Treatment Team. Using clinical experience and expertise, supporting both the treating team, consumers and families/carers to achieve their recovery goals.
This role requires a highly skilled communicator and the development and maintenance of strong relationships with key stakeholders both within Monash Health and externally. The position requires a broad understanding of the health care system, current and future workforce directions and industrial platforms. The incumbent is a role model of the core values and purpose of the organisation to ensure achievement of exceptional care and outstanding outcomes. The incumbent is a big picture thinker, an innovator, a mentor, a coach and someone who motivates and inspires others to meet all challenges presented.
This role is a full-time permanent position however it is open to internal applicants as a secondment
This role is open to the following disciplines: Registered Psychiatric Nurse Grade 5, Social Worker Grade 5, Occupational Therapist Grade 5, and Psychologist Grade 4.
About the Team
The Community Mental Health Continuing Care Team and Mobile Support and Treatment Services works across Clayton, Southern and Bayside regions to provide an effective and comprehensive service to consumers of the Clayton Adult Mental Health Program. Working within a multi-disciplinary team we provide treatment and support to individuals experiencing complex mental health and psychosocial challenges.
The Continuing Care Team Model of Care provide follow up to consumers with mental health issues requiring specialist mental health treatment and support that often is not met by generic services. This service provides case management support and treatment for consumers aged 25 to 65 years, operating Monday to Friday business hours, based at Atherton Rd Centre, Oakleigh.
The Mobile Support and Treatment Services is an assertive outreach services which provides case management to consumers aged between 25-65 years of age, with complex mental health needs who require psychosocial support and treatment. This service operates Monday – Sunday, working a rotating roster of morning and afternoon shifts, based at Atherton Rd Centre, Oakleigh.
