About The Role   

We are seeking passionate, experienced Dietitian to join our Proactive Support team at Monash Health Community. The successful incumbent is offered 0.6 EFT.  We are seek a dietitian with excellent communication skills to join the Monash Health Community Assessment and Response Team Directorate - Proactive Support and Managment team. The Grade 2 Dietitian is responsible for working within the team to provide best practice patient care, achieve maximal operational effectiveness, develop organisational competencies, and create strong partnerships within and outside Monash Health in collaboration with Senior Clinician Dietitians and Team Leaders.   

  

Essential to the role is experience within acute as well as an understanding of community services. The role works across various Monash Health sites. We operate from a person - centred care framework and align with the social model of health.

Monash Health Community proactively supports our community to improve and maintain health, independence and well-being by coordinating and/ or providing integrated, multidisciplinary care across multiple settings within the local community. Our aim is to empower and prepare clients to self-manage their health and health care.    

Working with clients across the aged spectrum, Monash Health Community provides a continuum of care, supporting clients through all stages of non-acute care from prevention through to rehabilitation. We provide services that support clients in their homes, community based facilities and dedicated Monash Health Community hubs.   

  

About you  

You are a dynamic clinician with a genuine passion and interest in this unique opportunity who has been developing your clinical skills and is ready to take this next step in your Career. You lead by example, thrive working in a dynamic environment and are truly passionate about delivering a high standard of evidence based care to all clients.    

You have sound skills in assessing and planning patient’s community health discharge requirements and have knowledge of the various community health support services available.  You are an advocate of Monash Health’s Strategic Guiding Principles

  • committed to providing a high standard of nutritional assessment, treatment through to discharge planning, and coordination of services to patients, to facilitate safe, effective, and person centred care.  

  • committed to delivering patient centred care and advocating for your patients.  

  • enthusiastic, compassionate and able to work collaboratively    

What you need  

Essential:   

  • An approved undergraduate degree in Nutrition and Dietetics from a Dietetics Australia (DA)  accredited course or an approved Bachelor of Science Degree (or equivalent qualification) followed by a postgraduate degree in Nutrition and Dietetics from a DA accredited course or The DSR dietetics examination for overseas trained dietitians, administered by DA.  

  • Current AHP registration or registration with National Registration Authority   

  • Valid Driver’s Licence   

  • Australian working rights    

Desirable:   

  • Previous Community experience.  

  • Previous experience working with individuals living with diabetes.
