About the Role
 
Join our team as a Grade 3 Social Worker on a part time (0.6 EFT) ongoing basis. This position will work from our community centre in Pakenham. 
 
In this role you will support people with complex mental health needs in the community. You’ll work in a caring, multidisciplinary team and help individuals on their recovery journey. Key responsibilities will include:
 
  • Provide mental health support in the community
  • Complete biopsychosocial assessments and create care plans
  • Offer one-on-one and group therapy
  • Collaborate with a team of clinicians and lived experience workforce
  • Supervise other social workers
  • Engage in discipline specific, professional duties- group supervision/ peer reflection/ discipline specific meetings and training
  • Work with families and the consumer’s community
  • Referrals to external and internal services to support and individual’s recovery
 
 
About Us
 
The ICP Team is part of Monash Health’s Adult Mental Health Program. We support people who often need frequent hospital care by offering flexible, person-centred services in the community. Our team includes social workers, psychologists, occupational therapists, peer workers, and more.
 
About You
 
You are a skilled and compassionate Social Worker with over seven years of clinical experience and a strong background in leadership within mental health and alcohol and other drug (AOD) settings. You bring well-developed assessment capabilities—including mental state, biopsychosocial, risk, and environmental assessments—along with expertise in crisis support and comprehensive care planning. 
 
Your professional practice is grounded in a solid understanding of statutory frameworks such as child protection and family violence, supported by thorough knowledge of relevant legislation. You excel in collaborative environments and demonstrate exceptional communication skills, enabling you to work effectively within multidisciplinary teams and support the best outcomes for clients.
 
Qualification & licence requirements include: 
 
  • AASW-accredited Social Work degree
  • AASW membership (or eligibility)
  • Victorian Driver’s Licence
  • Accredited Mental Health Social Worker status or working towards it
