Monash Health is seeking a passionate and experienced Grade 3 Social Worker to join the Intensive Care Package (ICP) Team on a part-time (0.6 EFT), ongoing basis. Based in the Pakenham area, this is a rewarding opportunity for a clinician who thrives in a complex, community-based mental health setting.
About the Role
This role supports consumers connected to the Pakenham Continuing Care Team who have complex mental health and psychosocial needs, often with high levels of acute service contact.
As a Senior Social Worker, you will:
- Provide assertive, recovery-oriented mental health support in the community
- Conduct comprehensive assessments and collaboratively develop intervention plans
- Deliver a mix of 1:1 therapy and group-based programs
- Work closely with a multidisciplinary team including OT, Clinical Psychologists, Peer Workers, AOD clinicians, Art Therapists, and Lived Experience professionals
- Provide clinical supervision to Social Workers within the Adult Mental Health Program
- Coordinate with families, carers, and a range of external services to support long-term recovery

About You
You're a qualified and highly experienced Social Worker with at least 7 years’ post-graduate experience, who is confident working with consumers presenting with complex clinical and psychosocial needs. You also bring:
- Proven ability to engage with consumers who experience frequent acute presentations
- Expertise in mental health assessment, risk management, and intervention planning
- A commitment to recovery-oriented, trauma-informed, and family-inclusive practice
- Skills in engaging with carers and service providers to co-design comprehensive care plans
- Demonstrated ability to provide clinical supervision and mentorship to other Social Workers
- Confidence working across complex service systems with multiple stakeholders
You will need:
- Bachelor or Master of Social Work (Qualifying) from an AASW-accredited program
- Eligibility for full AASW membership
- Minimum 7 years post-qualification experience
- Accreditation (or working towards) as an Accredited Mental Health Social Worker
- Postgraduate training (completed or in progress) in areas such as family therapy, trauma, or AOD
You will have up-to-date working knowledge of:
- Mental Health and Wellbeing Act 2022
- Family Violence Protection Act 2008, MARAM, FVISS, and CISS
- NDIS Act, Carer Recognition Act, and other relevant state/federal legislation
- AOD and forensic frameworks
- Recovery-oriented practice, supported decision-making, and cultural safety principles
