About the Role

Join Us as a Grade 3 Social Worker in the Intensive Care Package (ICP) Team!

 

We are looking for a passionate and experienced Grade 3 Social Worker to become part of our dynamic ICP Team on a part-time, ongoing basis (0.56 EFT / 44.8 hours per fortnight).

 

Based in the Pakenham area, this is a rewarding and impactful opportunity for a clinician who thrives in a complex, community-based mental health setting - supporting people when it matters most.

 

As a Senior Social Worker, you will:

  • Provide assertive, recovery-oriented mental health support in the community
  • Conduct comprehensive assessments and collaboratively develop intervention plans
  • Deliver a mix of 1:1 therapy and group-based programs
  • Work closely with a multidisciplinary team including OT, Clinical Psychologists, Peer Workers, AOD clinicians, Art Therapists, and Lived Experience professionals
  • Provide clinical supervision to Social Workers within the Adult Mental Health Program
  • Coordinate with families, carers, and a range of external services to support long-term recovery

 

This role supports consumers connected to the Pakenham Continuing Care Team—individuals with complex mental health and psychosocial needs, often with high levels of acute service contact. You’ll be part of a passionate, collaborative team working to empower consumers and strengthen recovery pathways in the community.

 

 

About Us

The MMC Emergency Department has introduced a new inter-professional team which brings together workers from mental health, alcohol and other drug (AOD) services, allied health & lived experience to provide specialised and integrated care to patients presenting to the ED with high acuity mental health, AOD issues, and complex psychosocial needs. This service is called the Mental Health and AOD Hub of which the EMAAP Team operates in, located in the emergency department. The team will provide a seven day a week, extended hours service, as well as an assertive outreach function that supports people following discharge for up to 28 days.  

 

 

The MH&AOD Hub Team is committed to providing a service that acknowledges an individual’s strengths and abilities, and is welcoming, accessible, responsive, collaborative and safe. The team seeks to develop positive relationships with people in crisis, and deliver care and support that respects the individual’s rights, preserves their dignity, and promotes recovery and inclusion.

 

 

About You

You are an experienced and compassionate Social Worker with a minimum of seven years' clinical experience, including leadership in mental health and alcohol and other drug (AOD) settings. You bring advanced skills in psychosocial assessment, recovery-oriented practice, crisis intervention, and care planning within complex healthcare environments. Your expertise is supported by strong knowledge of relevant legislation, including the Mental Health Act and Carer Recognition Act.

 

 

You thrive in multidisciplinary teams, demonstrating excellent communication, time management, and conflict resolution skills. You’re adept at navigating systems, collaborating with community services, and adapting to evolving healthcare demands. With a solid grounding in social work theory and ethics, you apply reflective practice, cultural sensitivity, and a commitment to quality care in all aspects of your work.

 

 

Qualification & Licence Requirements include:

  • An Australian Association of Social Workers (AASW) accredited Bachelor Degree or entry level Master’s Degree of Social Work that provides eligibility for membership of the AASW.
  • If tertiary education was completed overseas written evidence of AASW Assessment of Overseas Qualifications 
  • Current eligibility for AASW membership
  • A current Victorian Drivers Licence
  • Accredited Mental Health Social Worker:  approved by the AASW & a post-graduate degree in a relevant field or working towards (e.g. Family Therapy, Masters of Social Work, Mental Health, etc.) is advantageous.
